Testing Opt-Out Forms

Annual OSAS Opt-Out

Oregon law (ORS 329.479) permits parents/guardians and adult students* to annually opt-out of Oregon’s statewide summative tests in English language Arts (ELA) and/or Mathematics by submitting this form to the school the student attends. The school shall provide any student who is excused from a statewide ELA or mathematics test with supervised study time while other students are testing.

SEED Survey Participation

Oregon law (House Bill 2656) permits parents or guardians to decline, either verbally or in writing, to have their student participate in the SEED Survey. The school shall provide any student who is excused from participating in the SEED Survey with supervised study time (or another learning activity) while other students are completing the survey.

OSAS Science Exemption

Under Oregon Administrative Rule 581 – 021 – 009, parents may request that their student be exempted from taking the OSAS Science and/or ELPA 21 tests to accommodate a student’s disabilities or religious beliefs. To best support school planning, please complete and submit this form to Alsea’s Front Office by March 1st of the current school year.
